The solution is composed of an enterprise workflow dashboard that digitally connects with a consumer/patient facing mobile App. From the dashboard, enterprises manage, analyze, survey, message and educate their population. From the App, users create, control and own a permanent and portable health record, receiving medication alerts, tracking data, sharing information and records with caregivers and care providers, uploading important documents – such as insurance cards and advance directives – texting caregivers, and mapping family health. Personalized health education content called TEA (Targeted Educational Assistance) is delivered to the App.
