ICmed RPM enables providers and the qualified healthcare clinicians to remotely monitor their patients' blood pressure, blood-glucose levels, weight and more, and virtually send notifications, encouragement, or treatment adjustments when necessary.

Population Risk Stratification

Based on aggregated patient-generated data and additional relevant information, ICmed RPM provides a visually understandable report which provides a risk stratification view to providers, identifying patients who are at the greatest risk for a worsening condition or complications.

How It Works

Sign up

Providers can sign up to ICmed RPM quickly by providing basic details about their practice/health system.

Enroll and Invite Patients

Providers can import/invite their patients to the ICmed platform. The patients will receive invites and they can install the ICmed customer mobile app. Patients/Providers can also invite caregivers to the platform for increased communication and engagement. ICmed educates and assists patients with device connection.

Acquire Patient Data

Data from the patient’s mobile device is transmitted in real time to the enterprise dashboard. ICmed also sends alerts to patient and their caregivers for missing data entries to ensure the necessary RPM compliance.

Review Patient Data & Track Time

Instantly updated reports and analytics will be available in the provider portal. ICmed will automatically track the time utilized for reviewing data for each patient. Options to engage in interactive communications digitally with patient/caregivers is available in the provider portal.

Bill for Medicare & Get Paid

Reimbursement reports are generated for each patient, for every 30 day period, including the applicable CPT codes and the associated RPM review times. Medicare reimbursements can be easily filed using generated reports from the provider portal for each patient.

Remote monitoring of physiologic parameters (e.g., weight, blood pressure, pulse oximetry, etc) initial; setup and patient education on use of equipment.

Remote physiologic monitoring treatment management services, 20 minutes or more of clinical staff/physician/other qualified healthcare professional time in a calendar month requiring interactive communication with the patient/caregiver during the month.
